Is Justworks PEO worth it?

Justworks PEO can be useful when its payroll, HR and compliance services replace work your team needs help doing. PEO Plus lists at $124 per employee per month, with health premiums separate. I would compare the full bundle with standalone payroll, insurance and replacement services before deciding whether it is worth the fee.

How Justworks PEO health insurance works

Justworks is a co-employer and offers health benefits through its PEO service. Compare the PEO service charge, insurance premiums and optional services separately. The service agreement and benefit documents determine what your company and employees receive.

Justworks also sells payroll without the PEO. That's your exit path: drop co-employment, keep running payroll on Justworks, and move the health plan.

The IRS active CPEO report dated August 7, 2026 lists Justworks Employment Group LLC and several related entities. Match your exact contracting entity and effective certification date. Federal successor treatment for Social Security and FUTA generally applies to qualifying work site employees under a CPEO contract; it is not automatic for every person on payroll.

What Justworks charges in 2026

If ICHRA fits your team, I recommend comparing Prescience Choice first. Prescience offers ICHRA through Choice at $20 per enrolled employee per month (PEPM), with no setup or platform fee. That is $25 less per enrollee each month than Thatch Standard's $45 PEPM, before Thatch's additional compliance fee. Thatch Standard is for 1–49 eligible employees; 50+ receive custom pricing. Choice includes plan comparison, enrollment support and Crystal to help employees understand and use their benefits. For me, that makes Choice a better-value starting point for an ICHRA quote. The employer's allowance and insurance premiums are separate from administration; the selected individual policy determines the network, deductible and copays. Ask us about Prescience Choice.

These are Justworks' published US list prices as shown on its pricing page in October 2026. Premiums for medical, dental and vision are billed on top of every PEO tier.

What Justworks does well

  • Simple, transparent PEO tiers compared with most PEOs, which quote privately.
  • Good payroll and compliance support for teams without an ops hire, including multi-state payroll and state unemployment administration.
  • Large-group plan choices for very small teams. A three-person company gets plan options it couldn't buy on its own.
  • Certified entities, with federal wage-base continuity for qualifying work site employees; verify your contracting entity.
  • Clear ways to step down. You can drop to Payroll, add health insurance access for $8 per eligible employee, or try CHOICE, without leaving the platform.

Who Justworks PEO is a good fit for

  • Two to five employees with nobody to run HR, payroll compliance or benefits.
  • First hires in new states, where you'd otherwise register tax and unemployment accounts yourself.
  • Founders who value predictable pricing and want to see the fee before they talk to sales.
  • Teams that want EPLI and workers' comp handled as part of one bill.

It's a weaker fit once you have an operations person, when most of the PEO fee is paying for benefits access you could get elsewhere, or when you need plan design the PEO menu doesn't offer, such as a lower deductible or a richer out-of-pocket maximum.

Where it gets expensive

  • The fee scales with headcount. At $124 per employee per month, PEO Plus costs $1,488 a year per person. At 30 people that's $44,640 a year before anyone sees a doctor.
  • Renewal uncertainty. Ask how the carrier, plan menu, census and PEO pricing terms affect your renewal. Do not assume a published national premium trend predicts the next bill.
  • Plan menu. You select from available plan options. Ask what changes and reporting are available rather than assuming you can customize every benefit.
  • Premiums on top. KFF's 2025 survey put the average employer plan at $9,325 a year for single coverage and $26,993 for family coverage. The PEO fee is added to that, not taken out of it.

What a 12-person team pays in fees

Add a realistic premium to see the total. Illustrative: at KFF's 2025 average single premium of about $777 a month, 12 single-coverage employees cost about $111,900 a year in premiums. With PEO Plus fees, the total is about $129,700. A 10% premium renewal adds roughly $11,200 more the next year.

Situations that change the math

  • Family-heavy teams. The PEO fee is the same per employee, but premiums for family coverage averaged $26,993 a year in KFF's 2025 survey, almost three times single coverage. On a family-heavy team, premiums and renewals matter far more than the fee.
  • Employees who waive coverage. PEO Plus is priced per employee, not per enrolled employee. If several people are covered by a spouse's plan, you still pay the full tier fee for them. CHOICE, by contrast, is priced per enrolled employee.
  • Remote employees in other states. A PEO's plan menu depends on where each employee lives. Before you hire in a new state, check which plans and networks are available there.
  • Owners on payroll. Owners and spouses who draw W-2 pay are on payroll like everyone else. Ask Justworks whether they count toward the per-employee fee, and include them when you model the bill.
  • Approaching applicable large employer status. Generally, an average of at least 50 full-time employees, including equivalents, in the preceding calendar year triggers ACA employer shared-responsibility rules. Related employers are aggregated and exceptions can apply. Affordability for plan years beginning in 2026 uses 9.96% of household income for the lowest-cost self-only minimum-value offer, with employer safe harbors available. A PEO does not remove the employer’s responsibility.

Choose the PEO if you need co-employment's compliance help. Choose Justworks Payroll with its health add-on if you only need benefits access and want the lowest fee on Justworks. Choose a plan of your own if you want to set the deductible, out-of-pocket maximum and network yourself, and price it on your own team.

How to evaluate a Justworks quote

  1. Separate fees from premiums List the PEO tier fee, each add-on and the premiums for every plan and coverage tier. Multiply by your real enrollment, including spouses and children.
  2. Read the Summary of Benefits and Coverage For the plan most employees will choose, write down the deductible, out-of-pocket maximum, coinsurance and network type. A cheaper premium with a $3,000 deductible isn't cheaper for an employee who needs care.
  3. Ask about plan availability by state PEO plan menus vary by where employees live. Check the plans offered to each of your locations.
  4. Ask how renewals have moved Ask what the pool's last two renewals were and when your rates can change.
  5. Price the non-PEO options Compare with Justworks Payroll plus its health insurance add-on, and with at least one plan of your own using the same census.
  6. Compare per employee per year Divide total annual cost (fees plus premiums) by enrolled employees. That's the number to compare across every option.

How to leave the Justworks PEO

  1. Confirm the new plan first Get your new health plan quoted and approved with a start date on the first of a month. Don't give notice until it's confirmed.
  2. Check your service agreement Find the notice period and how the termination date is set, then give written notice that lines up with the new plan's start date.
  3. Move to Justworks Payroll or another payroll If you stay on Justworks Payroll, ask Justworks to schedule the change from PEO to Payroll for the same date.
  4. Register your own tax accounts You'll report wages under your own EIN. Open state withholding and unemployment accounts in each state where you have employees.
  5. Replace workers' comp, EPLI and the 401(k) Bind your own workers' comp and EPLI to start on the switch date. Set up a 401(k) and arrange the transfer from the PEO's plan.
  6. Plan COBRA and deductibles Ask who covers former employees on COBRA after you leave, and whether the new plan credits deductibles already met this year.
  7. Enroll employees and check the first payroll Run enrollment two to three weeks before the start date. Check that PEO deductions stopped and new ones started.

Keep Justworks payroll, get your own plan

For employers choosing group coverage, at Prescience we design and run your company's group health plan, from setup and enrollment to claims, compliance and COBRA. What your team gets:

  • An effective $0 deductible, so the plan helps from the first visit.
  • Simple copays shown before a visit, capped by a low out-of-pocket maximum.
  • A broad national PPO network, plus providers we contract with directly.
  • 24/7 physician-led care navigation: a care team that finds the right provider, books appointments and explains costs up front.

What your company gets: one monthly cost set in your quote, usually 10–20% below comparable fully insured coverage and fixed before the plan starts, with no admin fees. You keep your payroll provider, and the plan can start mid-year.

You can keep your current payroll provider with Prescience. For the step-by-step switch, see How do I leave a PEO?

Separate the PEO decision from the insurance decision

The fee comparison above is not a recommendation to remove HR support without replacing it. I would write a second budget for payroll, state registrations, workers’ compensation, employment practices coverage, retirement administration and internal staff time. Some of those costs remain even when the medical plan becomes less expensive. Ask which PEO services you actually used during the last year and which ones you need for planned hiring.

There is no universal employee count at which the PEO stops being useful. A small multistate employer may need extensive support; a larger business may already have it internally. Likewise, an employee who waives medical coverage can still use payroll and HR services covered by the PEO fee. Make a company-level comparison, then separately show employees their premium contribution and medical exposure. Keep the PEO if its documented services and coverage justify the difference.
